Wellness Fund - Apply OnlineYou may apply to the Wellness Fund online, but it is the handler’s responsibility to ensure that all supporting documentation (including receipts and the Veterinary Form) are submitted to GDUC. Your application cannot be considered until all three parts - your application, your vet’s information form, and your receipts or estimates - have been submitted. Application Forms
