The applicant or veterinarian can apply online to the Wellness Fund (see ‘apply online’ link), or by downloading forms (see ‘Application forms link) and submitting them to GDUC Wellness in these ways:

Please ensure that application forms are completed legibly. The information on this form is are necessary to evalate the application.
Please also remember that all supporting documentation MUST be submitted by the applicant and/or veterinarian as soon as possible.
Wellness Fund applications are normally processed and evaluated within two weeks.
