The applicant or veterinarian can apply online to the Wellness Fund (see ‘apply online’ link), or by downloading forms (see ‘Application forms link) and submitting them to GDUC Wellness in these ways:

Please also remember that all supporting documentation MUST be submitted by the applicant and/or veterinarian as soon as possible. Applications should be submitted electronically as the committee is comprised of GDUC volunteers who are either partially sighted or completely blind.
